Insurance Verification

The first step is to determine your insurance coverage and if nutrition consultations are covered. Fortunately, many insurance companies do cover nutrition visits with a registered dietitian. Please fill out the insurance verification questionnaire and submit to us by emailing the completed form to nutrilife.RDN@gmail.com or fax to 626-816-3697. This step needs to be completed prior to obtaining a referral from your doctor. You can call the 1-800 number on the back of your insurance card to help you fill out the questionnaire.

RD Referral Form: Medicare Insurance

We are currently accepting original Medicare Insurance! For nutrition services covered through Medicare, you must have a referral from your healthcare provider (MD or DO).
Only nutrition counseling for Chronic Kidney Disease (CKD) stage 3, 4, 5 (non dialysis), Diabetes Mellitus Type I and II, and post kidney transplant within 36 months are covered through Medicare insurance. No other medical diagnoses are covered.
A total of 3 hours in the first year and 2 hours of nutrition counseling are covered, whether nutrition counseling was provided by us or another registered dietitian.
Please provide the referral form below to your Medicare doctor to complete prior to scheduling your visit with us.

RD Referral Form: Private Insurance (PPO and HMO)

We currently accept the following insurance carriers:
1. United Health Care
2. Aetna3. Optum
4.Cigna
We are currently working to partner with multiple private insurance companies at this time and will update this section (Hopefully sooner than later!) We also accept private pay clients at this time as we establish our practice.If you have HMO insurance, you will need to obtain a prior authorization prior to your visit with us. A prior authorization is an approval for you to see a provider of your choice. When you visit your medical provider or your PCP, you can ask the staff to submit a prior authorization for you for your nutrition consultation.

After you make an appointment, our patient portal ( Practice Better) will send a link to your email asking you to complete an intake form, medical history, eating habits, and preferences. We will gladly answer any questions or concerns about completing the forms.

Payment

A credit card will be required to be put on file to hold your appointment. We know there are changes in plans in life. Thus, there are no charges for 24-hour advance cancellation or rescheduling. However, credit cards will be charged a percentage of the service fee in case of late cancellation or no-show, regardless of the reason.

Medicare
Medicare only covers medical nutrition therapy if you have a diagnosis of Diabetes or Chronic Kidney Disease stage 3, 4, or 5 (pre-dialysis)
Medicare-covered visits/hours:
3 hours for the first calendar year
2 hours for subsequent calendar years
Additional visits may be requested, and a new referral from your doctor will be required.
Don’t hesitate to contact us with any questions regarding your Medicare coverage.
Note: Please have your MD/DO referral form before your appointment. If you fail to present your referral form 72 hours before the appointment, we may decline/cancel your appointment request and open the spot for other clients.
